Volunteer forms are available on the E-AYSO website at http://www.eayso.org

FAQ About Volunteer Forms:

• All volunteers who come in contact with youth players or youth referees must fill in a Volunteer Form.  We crosscheck our database with the Volunteer Form database and if you have not completed the form you may not serve in a youth contact volunteer capacity (that is an AYSO mandate).
• Youth Referees and Youth Coaches must complete a Youth Volunteer Form.
• New Volunteer Forms are required each year.
• First time users will need to establish a username and password.
• The Volunteer Form must be completely filled in. All references must be complete: Name, address and phone number. All appropriate boxes must be checked
• Social Security Number and Driver's License (or appropriate, acceptable ID) number are required--we cannot accept your Volunteer Form without both.
• Print three copies of the form, keep one for your records and either mail or drop TWO COPIES  of the form to:
